2024-2025 School Year: A Message from Dr. Melissa Hocate:

Dear CSS Families,

I hope you all have had a wonderful start to the new school year. It is a joy to see students back on campus and our beautiful campus buzzing with energy as students and families re-established and made new connections with each other. We have unfortunately learned of a few reports of positive COVID cases and family exposures in our CSS community. Locally and nationally there has been a significant rise in cases. I wanted to notify you of this so that you can monitor for symptoms, test, and stay home if you are not feeling well. Official guidelines from the CDC have changed over time and can be found here. The most important is to test and stay home if you are not feeling well with symptoms suggestive of COVID which include headache, congestion, runny nose, loss of taste or smell, body aches, fatigue, nausea, vomiting, or fever. Masks, disinfection, ventilation, and isolation of positive cases help mitigate the spread of COVID and other respiratory illnesses. COVID at-home tests can be purchased at various retailers or online. New COVID boosters are also available and help fight the latest strains of COVID. Routine cleaning of classrooms is daily, and enhanced cleaning protocols are in place when cases are discovered.

Mandatory Medical and Health Information:

To comply with State and local regulations, The Colorado Springs School must maintain each student's current health and immunization records. Forms and documents will be accepted in the Trianon Building Business Office.

School and sports physicals are required to be current for a 12 month period. Students must have a current physical exam form and an immunization certificate on file in the Infirmary. This is required before the first day of school each year to attend classes and participate in sports. Questions reach out to the Infirmary: Campus Health Coordinator Coordinator at (719) 434-3506.

Mandatory medical and health records can be found below. Using your login credentials, guardians are responsible for completing their child's medical paperwork. The following forms may require a physician’s signature and must be submitted to the Health Center before the school begins.

  • Annual Physical Exam Form (Preferred Form, applies all ages) This form is good for 365 days from the date of the last physical
  • School Required Immunizations: Required immunizations for the school year are mandated by Colorado state law. Learn more here for information/resources, including: Child Care Immunization Chart & Certificate of Immunizations: Read Here
    • Colorado law requires students who attend a licensed childcare, preschool or Head Start program to be vaccinated against many of the diseases vaccines can prevent. Your student must be vaccinated against: diphtheria, tetanus & pertussis (DTaP, DTP), polio (IPV), measles, mumps, rubella (MMR), hepatitis B (HepB) haemophilus influenzae type b (Hib), pneumococcal (PCV), varicella (chickenpox)
  • Permission to Treat: Login in to your FACTS account, a completed form must be on record. (listed under Web Forms, then Over-the-Counter Medications Form). Certain over the counter medications can be offered to students to alleviate certain symptoms on and off campus if this form is completed.

If your child needs physician-prescribed medication on campus, the Campus Health Coordinator can coordinate the administration of that medication during the school day. Please complete and submit this form:

Off-Campus Permission to Medicate Form

When students are traveling on a school trip medications need to be dispensed under supervision age depending. A completed form must be submitted prior to trip based on the age of the child. This includes any Experiential Education trips (i.e. Fall Adventure or Colorado Expedition, Seminars, ECS, Fall Adventure or Colorado Expedition). The Campus Health Center will collect the medication prior to the trip. Please complete the applicable forms for the age of your student:

2024: Updating Student Medical Information

To update and submit the Student Medical forms, Care Plans, and/or Over-the-Counter Medications form on file, please contact the Infirmary at (719) 434-3506. Forms will be accepted after July 15th, 2024 in-person or by mail (attn: Infirmary). Or via Email to Dr. Hocate

ATHLETICS: CHSSA - 2024-25 REQUIRED MEDICAL FORMS (UPPER SCHOOL ONLY)

CSS is a member of CHSSA (Colorado High School Activities Association) and as a member school they require both forms: annual school physical and updated Preparticipation Eligibility Form (PPE). Please review our 2024-25 forms and policies here for eligibility in CSS athletic programs:
